Digital trichoscopy and trichogram: assessment of the scalp and hair

Digital trichoscopy is an exam that magnifies the image of the scalp and hair with a dermoscopy camera, at the clinic with the FotoFinder® system, to identify the cause of hair loss. It shows hair thickness, density, signs of inflammation and patterns typical of each type of alopecia. Combined with the trichogram (hair analysis), it allows images to be compared throughout treatment. At Yamagata Dermatologia, in Ipanema, Rio de Janeiro, it is performed by dermatologists.

How it works

The digital dermoscopy camera magnifies the scalp and shows, on a monitor, details that are not visible to the naked eye. The doctor and the patient can view the images together, and they are stored for comparison at later appointments.

Trichoscopy assesses, among other aspects:

  • Hair thickness: variation in thickness and miniaturization (progressive thinning of the hair) are typical signs of pattern hair loss.
  • Density: the number of hairs per area, which can be measured and compared over time.
  • Scalp: signs of inflammation, scaling, vessel changes and patterns characteristic of each type of alopecia.

The trichogram complements the assessment with analysis of the hair cycle, when indicated.

What it is used for

  • Identifying the cause of hair loss and distinguishing the types of alopecia.
  • Assessing the severity and extent of the condition.
  • Monitoring the response to treatment with comparable images.
  • Guiding the site of a scalp biopsy, when one is needed.

What the exam is like

The doctor parts the hair in different regions and places the camera on the scalp to capture the images. Global photographs of the head and, when indicated, a trichogram may also be taken. The exam is painless and takes about 1 hour, including the appointment.

Preparation

  • Wash your hair one to two days before the exam.
  • Do not use creams, oils or styling products on the day.
  • Avoid hair dye and chemical treatments in the preceding days, unless instructed otherwise.

Results and follow-up

The result is discussed at the appointment, together with the treatment plan. The images are repeated throughout follow-up to compare hair density and thickness.

Limitations

Trichoscopy does not replace a complete clinical investigation, which may include blood tests, or a biopsy in cases where one is needed, such as scarring alopecia.

All procedures are recommended and performed after an individual medical evaluation. Results vary from person to person.

Questions

Frequently asked questions

What is trichoscopy used for?

To identify the cause of hair loss or hair thinning, distinguish the types of alopecia, assess severity and monitor the response to treatment with comparable images.

Does it require preparation?

Wash your hair one to two days before, do not use creams, oils or styling products on the day of the exam and avoid recent hair dye, which can affect the assessment. The doctor may give specific instructions.

What is a trichogram?

It is the analysis of a small number of hairs, pulled out with tweezers, which shows which phase of the growth cycle they are in. It helps assess the intensity of shedding in some cases.

Does trichoscopy replace a scalp biopsy?

In many cases, trichoscopy is enough for the diagnosis. In scarring alopecia and doubtful cases, a biopsy may be needed, and trichoscopy helps choose the best site for it.
