This policy explains how Yamagata Clínica Médica e Cirúrgica Ltda (Yamagata Dermatologia), registered under CNPJ 27.955.591/0001-07, located at Rua Visconde de Pirajá, 142, suite 1401, Ipanema, Rio de Janeiro – RJ, 22410-000, Brazil, handles personal data sent through this website, in accordance with Brazilian Law No. 13,709/2018 (General Personal Data Protection Law, LGPD). This is a translation of the Portuguese version; in case of any discrepancy, the Portuguese text prevails.

What data we collect

The website collects personal data only when you fill in the contact form: name, phone number, email (optional) and the subject of your request. The form does not ask for, and should not receive, information about your health, which is handled only at the appointment, in the medical record.

The website does not use advertising cookies or visitor tracking tools.

How we use the data

The form data is used exclusively so that the clinic's team can contact you to book or clarify the requested appointment. It is not used for advertising, nor sold or shared with third parties.

Legal basis

Processing is based on your consent (article 7, I, of the LGPD), recorded when the form is submitted, together with the date and the version of the authorization text.

Who the data is shared with

The data is stored on technology services contracted by the clinic to host the website (Vercel) and to store contact requests (Supabase), which act as processors and follow the clinic's instructions. Access to the requests is restricted to the team responsible for booking.

How long the data is kept

Contact requests are kept for as long as necessary to respond and are deleted after 12 months, unless the law requires otherwise.

Your rights

As the data subject, you may, at any time and free of charge:

• confirm whether the clinic processes your data and access it;
• correct incomplete, inaccurate or outdated data;
• request the deletion of data processed on the basis of your consent;
• withdraw your consent;
• obtain information about who the data has been shared with.
