Facial contour and double chin: jawline definition without surgery
A double chin and loss of facial contour definition result from a combination of fat under the chin, skin laxity, loss of jawline support and, in some people, a weak chin. Non-surgical treatment identifies which of these factors predominates and combines technologies that reduce fat and tighten the skin with injectables that redefine the contour. At Yamagata Dermatologia, in Ipanema, Rio de Janeiro, the plan is defined by a dermatologist at the appointment.

Why it happens
The definition of the facial contour depends on skin firmness, the amount of fat under the chin and the bone structure of the jaw and chin. With aging, the skin loses firmness, the jaw loses volume and fat accumulates and descends, which softens the line separating the face from the neck.
Genetics also play a role: a weak chin or a short jaw favor a double chin even in young, thin people.
How the evaluation is done
The dermatologist assesses the face from the front and in profile, the amount of fat under the chin, skin laxity, chin projection and the strength of the masseter and neck muscles. Based on this, they define the weight of each factor and the treatment plan.
Recommended treatments
- Ultraformer® MPTContour firmness and reduction of small fat pockets under the chin.
- Evolla® MPRadiofrequency for firmness of the double chin and contour.

Combined treatments
Treatments are often combined: Ultraformer® or radiofrequency act on the double chin and firmness, fillers project the chin and define the jawline, and botulinum toxin relaxes the muscles that pull the contour down.
All procedures are recommended and performed after an individual medical evaluation. Results vary from person to person.
Questions
Frequently asked questions
Is a double chin just fat?
Not always. A double chin can be formed by fat, skin laxity, a weak chin or a combination of these factors. Identifying the main cause is what defines the treatment.
Do thin people get a double chin?
Yes. Genetics, jaw shape, posture and laxity can cause a double chin even in people at a healthy weight.
Can a double chin be treated without surgery?
In mild to moderate cases, yes. Technologies such as Ultraformer® and radiofrequency, combined with chin and jawline fillers, improve definition. In cases with a large excess of skin or fat, surgery may be more appropriate.
Does jawline filler make the face look masculine?
No, when the plan respects the features of each face. On a female face, the goal is usually a smooth, defined transition; on a male face, a more marked jawline.
