Gummy smile: treatment with botulinum toxin
A gummy smile is excessive gum exposure when smiling, usually more than three to four millimeters. When the cause is the strength of the muscles that lift the upper lip, botulinum toxin, applied in small doses, reduces this lift and decreases gum exposure, without affecting the naturalness of the smile. At Yamagata Dermatologia, in Ipanema, Rio de Janeiro, it is performed by dermatologists after a medical evaluation.

Why it happens
A gummy smile can have different causes: overactivity of the muscles that lift the upper lip, a short upper lip, excess gum tissue, short teeth or greater vertical growth of the upper jaw bone. Often, more than one factor contributes.
How the evaluation is done
The dermatologist assesses the smile at rest and in motion, measures gum exposure and identifies whether the cause is muscular. When there are dental or bone factors, the patient is advised to seek a dental evaluation.
Recommended treatments
- Botulinum toxinReduces the lift of the upper lip when smiling.
- Lip fillerA complement for thin lips, which accentuate gum exposure.
Combined treatments
When the upper lip is thin, lip filler can complement the toxin by increasing gum coverage. Dental factors are treated together with the dentist.
All procedures are recommended and performed after an individual medical evaluation. Results vary from person to person.
Questions
Frequently asked questions
Can every gummy smile be treated with toxin?
No. The toxin works when the cause is the strength of the lip muscles. When the cause is large gums, short teeth or the position of the bones, treatment involves the dentist or the oral and maxillofacial surgeon.
Will my smile look artificial?
No, when the dose is appropriate. The goal is to reduce gum exposure while keeping the natural movement of the smile.
How long does the effect last?
On average, four to six months, varying from person to person.
Does it hurt?
The application is done with a very fine needle, at a few points, and the discomfort is mild and brief.
